-WOW rules knowledge LVL.
-Player Management LVL.
-Events you judged in, and the names of PTOs or stores you worked with.
-Will you be requiring a hottel room? Yes/NO
-Let us know what days you are going to be available to work at the DMF
-Let us know if you are interested in taking any tests: PM/WOW rules of level 2 or higher.
