It's not possible for TOs to delete locations. Send an email to TO@upperdeck.com and ask for the extra ones to get removed.

It's likely that this person is just running non-sanctioned tournaments. This doesn't require a UDE number from the TO. In fact, I think it doesn't require a TO at all.
The process for sanctioning a tournament requires for the TO to have a UDE number and a password. If the TO does not have one (or doesn't remember it), then they can't ...
